Students who enroll in DALLAS TeleCollege courses which require monitored (proctored) testing and who do not live in the physical service area of district colleges must nominate and gain prior approval for a proctor who will monitor course tests. Students living within the Dallas area do not need to submit this form; instead they should take their tests at a DCCCD Testing Center as detailed in the course catalog.
Proctors
:A proctor is an individual who agrees to receive your tests from the DALLAS TeleCollege, administer them to you in a suitable environment and return them to the DALLAS TeleCollege. They also certify in writing to the DALLAS TeleCollege that you completed the test according to all of the specific directions provided. As the nomination form indicates, a proctor will identify the specific tests s/he will monitor. If you use more than one proctor, you will need approval of each one. Any proctor you nominate must be acting in the official capacity in one of the following positions:
NOTE: Relatives, regardless of their position, are not permitted to be proctors.
Arranging for a Proctor:
You are responsible for identifying, nominating and making all of the arrangements for
the proctoring of your course tests, including the payment of any fees for services or the
return of test materials to the DALLAS TeleCollege (cost of fax, U.S. mail or UPS). If you
are unable to find a suitable proctor, please contact your DALLAS TeleCollege advisor for
assistance immediately. Proctor arrangements must be completed
within 10 days after the course begins.
Completing the Form:
Please use the Proctor Nomination form for
nomination and approval of your proctor. You will need to complete
the top part of the form for each course you are taking, even if you are using the same
proctor for all tests. Please be sure that you identify any and all
special testing equipment or conditions related to your course so that the proctor is
aware of what will be necessary and who will supply the items. For example, a course may
require that you view a portion of video, listen to a segment of audio tape or use a
calculator as part of the test. This information is contained in the course description as
well as in the syllabus.
The proctor must complete the remainder of the form, have his/her signature notarized, and
submit it to the address provided. You should supply your proctor nominee with an
addressed, stamped envelope if they are returning the form by mail.
Timeline:
The form should be completed and sent to the address below no later than 10 days after the beginning of the course.

If your proctor agrees to and is able to administer all tests for the course, you will not need to submit any additional forms for the duration of the semester. If problems arise with your proctoring arrangements, please let your course instructor know.
What happens next:
The form will be forwarded to the instructor. Once the instructor approves the nomination, s/he will forward testing materials with specific administration instructions to the designated proctor. It is the proctor's responsibility to return the materials to the instructor for grading. It is the student's responsibility to cover the costs to return the materials. It is preferred that testing materials be returned via certified mail.
